Anal Fissure - Infant Withholding Gas

I think I have done my homework. My 4 month old daughter was just "incidentally" diagnosed with an anal fissure. We went to the doctor because I was concerned about the color and strong odor of her stool. Fortunately, while we were in the presence of the doctor, my daughter had what we refer to as a "gas attack." Shortly before passing gas she starts screaming and tenses her whole body. Her screaming continues even after the passing. This prompted the doctor to examine her anus. We are now applying Bactroban ointment 3x/day. (Today is our first full day.) My daughter is breastfed and given one two, four or six ounce bottle of Carnation Good-Starts formula per day. When she was about three months old, the frequency of her BMs decreased. We had one especially bad day when 48 hours went between BMs. We thought this was normal and didn't think she was constipated. Now she has BMs every 24 to 36 hours. Sometimes she seems as if she is straining, other times she gives no outward signals that she's having a BM. It seems like she's ok, for the most part, with the BMs and that she's just withholding gas. Could she be constipated? Also, would the Americaine ointment you suggested for the 9 month old before BMs help our daughter. We have been giving her Mylecon 5x daily for about two months because we thought that her gas problem was in her stomach.
