Hi, Mike.
Since you have been examined by 3 ophthalmologists your problems are evidently beyond their experience. That means that it is time to stop going and start trying stuff.
The first thing I would try is simply drinking at least 8-8 ounce glasses of water every 24 hours. Let us know what you discover in a few days.
Your symptoms, in the absence of opinions by those docs, are most clsasically those of dehydration.

here's something else that's kinda different...
I had problems with my eyes too, and I found myself squinting all the time to try to see better. Things were fuzzy and sometimes it looked like there was a "clear coating" over them. Well, recently I was taking the homeopathic remedy kali iodatum for my swollen thyroid, and about mid-treatment, I realized that my eyes were suddenly open much larger than they had been before, and the fog had gone away. Went back and looked and this remedy is also key for frontal sinus problems.
So, as Walt says, start doing -- maybe see a practitioner in chinese medicine (acupuncture, herbs), or a homeopathic practitioner. It's time to move on and try to find something that will help.
