I have heartburn and I feel like I can't get enough air in my lungs. Sometimes I wake in the night and feel like I don't know how to breath. Once in a while I'll be able to get a good breath, but the feeling that I'm not getting enough air persist all the time. The doctor's are treating it as GERD, but it isn't helping. My chest and ribs hurt because I'm always trying to breath deep. What can this be, are the doctor's missing something?

YAH! He is missing the whole thing about what causes both of these.
This has been discussed many times right here on this bb. Go to the archives & read everything about GERD & you can't miss why you have the breathing problem too. By the way, it is called intercostal muscle bracing and costochondritis secondary to that bracing (which also happens to be the most common cause of Leaky Gut Syndrome--almost the only cause of GERD).
Once you have read all about it, if you still have questions, write again. In the meantime, you could get rid of the symptoms of the breathing by seeing a Certified Rolfer in your area AND the GERD symptoms by going to the FAQ page of this 'site & reading about upper intestinal symptoms.
