Please help

I wrote a couple of times about pain in groin. Well after cats and ultra sound including vaginal I need a little help understanding. During cat It found trace amount of fluid around left ovary. During vaginal ultra sound could not find left ovary at all! But did find a mass inside right ovary. I Have a history of endometriosis that has left me infertile. The exact words on lab report from the radiologist reads." Hyperechoic right ovarioan mass not incompatable with endometrioma. but could represent a hemorrhagic cyst. No other abnormalities are noted outside of nonvisualization of left ovary."
Could this be endo again I am being sent to a specialist. My Gp said probaly surgery. Could you help me with thr wording of the report. Could I have endo? Has it been ruled out? What about this cyst?

You more than likely have an endometriosis tumor that will have to be surgically removed. My advice would be that you have a "Complete Hysterectomy" performed rather than a partial hysterectomy. I say this because if they only perform a partial hysterectomy, within a years time, you will be back in for surgery on the other ovary. This way, they get paided for performing two surgeries rather than one. I know this because I've seen it happen more than once! * S.H.A.P.E. *

I know she said infertile, but I have a cousin who was and boom in her late 40's she had a boy.

Hysterectomy would not bother me at all if I know I wont be doing this again. I am 38 and have adopted three beautiful babies. My girls are seven and my boy three. I want to be healthy and active for them. So if I should have it all taken I most certainly have no problem with that. I am just concerned about hormones and side effects ect,,

Hi Amy, Usually only the growth and/or the ovary is removed. Unless you have severe complications the Uterus is not removed. If you have your uterus and ovaries out you will need hormones.

Hi, Amy.

IF the diagnosis in the past was correct, & sufficient endometriosis to cause the scarring necessary to produce infertility was already present, this is almost certainly endometiosis. Surgery is likely to cause, ultimately, as much scarring as what is going on and surgery will not likely do anything about your current symptoms. Dealing with the causes, even now, would not only get rid of your symptoms but would prevent any progression of this which has already crippled you (the infertility). Then, all you would have to cope with would be the scarring already present.

To totally remove the endometriosis, surgically, at least all of the pelvic organs (Bladder,sigmoid colon and rectum, etc.) would have to be removed and STILL a few cells of the "condition" would likely be present and recurrence on the organs you have left would be almost inevitable.

Dr. Walt are you saying even if I have this surgery I am still going to feel the pain in my groin and legs ect.. That's not sounding real yippy! How might I help the healing process? I am also anxious about going under the knife. I am having dreams I wake up while in surgery , I even panic while wide awake. I have never had this fear before. I am afraid I am going to get on the gurney and just emotionally break down.

In my opinion, there is a chance that definitive surgery might relieve some of your pain temporarily and could be worth going through while you are seriously dealing with the known causes.

I would like to thank all that have given me words of advice understanding and encouragement. When a person is going through a medical crisis it is a very scary, stressful and difficult time. We all want immediate answers even though they are not always available. It is comforting to know that we can log on, vent and get a response. Amy, I simply went through a similar type of presentation of symptoms. It really was a confusing experience for me. It does sound as though your previous endometriosis may be progressing.

In my case, an endometrioma was suspected. But this was a huge red herring. Indeed, my pelvic exam was not indicative of any type of cyst. My ultrasounds found a huge cyst on my left ovary that appeared to be hemorrhagic or an endometrioma and a normal right ovary. Three days after my ultrasound, my CT scan indicated what appeared to be a 5 cm endometrioma on my left ovary and a 4 cm cyst on my right ovary. However, upon bilateral laparoscopic ovarian cystectomy, my Ob/Gyn found that these were not single masses on each ovary as all other screenings indicated but several centimeters worth of numerous follicular cysts on each ovary.

I certainly understand your fear of going under the knife. I too was a complete wreck before my surgery. However, it really was the only way of making a definitive diagnosis and, in fact, the screenings I had prior to my surgery yielded a red herring, so the surgery was important both diagnostically and therapeutically.
