bladder infection

Hello Dr. Stoll,

For the past two months I have been dealing with a bladder infection. I have not had any burning, however do have frequent urination as well as a very strong chemical type odor and blood in my urine and ecoli was present in the culture that was taken. For the past two weeks I have been reg. bleeding all the time, similiar to a monthly menustral period which I normally would not have this for two weeks and never had one like this before but have had to have female surgeries in the past due to growths found that were benign so this is a question here as to the blood and where it is coming from at this point and time with the reg. bleeding I would say. Anyway I was first put on Cipro for one week 2 x a day, as soon as I stopped the bladder infection returned in a few days again. I then was put on another antibiotic but then was changed over to Levaquin 500mg for one week again it returned as soon as I stopped taking it. I am once again put on Levaquin 500mg for 2 weeks this go round to see if it will rid the bladder infection are not for good and if not my primary care is going to send me to a urology clinic. My question is, is this normal for ecoli to not respond in this length of time to antibiotics described? How would ecoli get into the urine and bladder? Can this be life threatening? Would a bladder infection going on for this amount of time cause bleeding such as I am doing now? I would really appreciate any information you can provide me with concerning this and any alternatives you may suggest that I try. I am not sure, my primary came in after looking at my urine under the slide as well as doing the test strip test in his office and saying blood was present and it showed I had a bladder infection and put me on the Cipro at that time and he sent it off for the culture to later call me at home letting me know it was ecoli but did not let me know about the colony counts. I was not aware to ask that has I have never been bothered with bladder nor kidney type infections in the past. Is there anything else I should be asking? Are any other information you can give to me in regards to my first posting? Thanks, Tootie

First of all, laboratories do not DO cultures without reporting the colony count. Up to 100,000/cc is considered normal ESPECIALLY in women. You need to have a copy of that report in your home.

Test strips are notoriously inaccurate in making a definitive diagnosis in cases like this.

The answer to the above will determine what I think--regarding your questions.

I called his office today and asked for the colony, the nurse said ecoli came back on the culture from the lab. I asked no I want the exact colony count, she said 100,000 and for me to continue taking the Levaquin for the next two weeks as perscribed and they would reculture when I came back in again. So if this is a normal range why are they telling me I have a bladder infection and the ecoli is present? What do you suspect is going on with me?

Hi, delores.

The female urethra is very short and so the germs can easily crawl up from the perineum that is heavily contaminated by e-coli.

Live threatening when a serious infection becomes chronic and ascends to the kidneys for years. Actually, the most common cause of renal transplant and dialysis is chronic e-coli infection of the kidneys.

It is hard for me to make recommendations from here. 100,000 colony count is just at the border between not significant and significant. If it were me, I would consider using mannose to resolve this since it is totally non toxic and the problem is really borderline.
