Question from a colleague reg. Mg-chelates

Dear colleague,

You often give advice to the patients on the board regarding the connection between extrasystoles, either VES or SVES, and intracellular magnesium levels. Perhaps you could answer, for the benefit of my own patients, a few questions without referring me to the archives, which are slightly confusing (the IV protocol is clear to me). Perhaps I can implement some of it in my own practice, since I am trying to limit the use of ß-blockers for those cases where the uses of these drugs are justified.

1.Which test is considered to be most reliable for intracellular magnesium and which lab (pref. in Europe) would do such a test?

2.Is there a risk that either oral or IV administration of magnesium alone disturbs the balance with calcium? IOW is there a need for simultaneous administration of calcium and if so what proportion Mg to Ca would you advice?

3.Which form of magnesium would be best absorbed (bioavailability)? There is some controvery lately regarding orothates, though.

4.Would one advice to orally take the Mg-chelate on an empty stomache, before a meal?

5.What about side-effects (other than diarrhea with oral suppl.). Have you ever noticed any in your practice (some patients complain about a burning feeling or tachycardia during IV infusion)

In order of your very good questions:

1. WBC or RBC magnesium tests. All labs will eventually do this test. The question is: how long have they been doing the tests and how many do they do every day? Until this is commonly done the results will still be questionable. Remember that a therapeutic trial is still the most accurate evaluation.

2. Not in my experience. I have never worried about the calcium ratio and never had the first indication of a problem.

3. I use magnesium glycinate, exclusively, but have found orotate and aspartate to work well. I have never heard of or had a problem with any of these. For IV, I use Magnesium cloride.

4. With a meal.

5. ALL IV symptoms are transient and can be alleviated by slower infusion. In my experience they are rare and nothing to worry about.

Hope this helps. Let us know what you learn and how this works for your patients. Do not forget that stress storage is the other most common cause of this.
