What started with excessive callous formation on the heels two years ago, has evolved into disabling/sometimes cribbling keratoderma of the feet. The palms of my hands are minimally involved. I have tried everything topical including creams and retinoids. Oral Accutane at low doses for three weeks actually helped but the systemic skin dryness was horrendoous. I have stopped that medication and the skin fissures are worsening again. I can barely walk and sleep at night is almost impossible from the pain and itching. Am I alone?

Since you aleady know it is coming from inside you, you must know that topical things cannot be the solution.
Start with at least 3000 milligrams of omega 3 fish oils twice a day. While you are waiting for that to work (about 3 months) you can use anhydrous lanolin (sparingly and rub in well) 2-3 times a day. Ask your pharmacist for a 2 oz jar (It should cost no more than a few dollars and last you the whole time.)
