The corners of my mouth have been badly chapped and fissured
for what seems to have been two months now. Some of the research I've done on the web would indicate that this is actually a form of candida infection that is called perleche. If so, are any of the over the counter antifungal preparations effective for this or do I need to see a doc. for a prescription?

Hi, Steve.
Actually, the most common cause of this is a relative deficiency of B complex vitamins. You might first try taking at least 100 Balanced B twice a day for a few weeks and see what happens.
ALSO, depending on the part of the country you live in, this could be aggravated by low indoor humiditiy.
If this is candida, you have it throughout your entire intestinal tract; the lips being the beginning of that tract. Only a physician knowledgable about C-RS would be competent to manage this.
You will find a lot of information about LGS and C-RS in the archives for this BB.
Let us know if you need more information.

Steve,
I had this type of condition myself for well over two months. I followed Dr. Stoll's advice on the B-vitamins, and it has cleared up within ten days! I didn't think I could possibly have a B vitamin deficiency since I've been eating a whole foods, metabolically appropriate diet for several months now. I've also been doing oral chelation for mercury detox, post amalgam removal, and perhaps this, along with other diet changes, has sponged up my B-vitamins....Thank-you, Dr. Stoll! Lori

Thanks, Lori.
This is not the answer to every case of perleche but surely the majority. I am glad it worked so well for you. Your "surprize" tells me that you are still learning about nutrition just like the rest of us.
Hopefully, your testimonial will help others become students of their own "wellness".

Dr. Stoll:
I have had red, uneven skin with very small white heads at the corners of my mouth for about 7 months now. I took "100 B complex" vitamins twice per day at your suggestion for two weeks now but am not sure if it has helped clear up the problem.
Are there foods I also need to be eating or staying away from? Other suggestions? Would a Vitamin B deficiency be related to a yeast infection or are they completely different? Also, how would I find a physician knowledgable about "CR-S management?"
Thanks for your help.

Hi, Pete.
First, it will take at least 4 weeks for the perleche to be cleared IF it is caused by the B vitamins not being high enough for YOU.
Next, call (800) 522-4762 for the names of physicians in your part of the country who would know enough to order several purged stool specimens AND what to do about the results.
Remember, your lips are the beginning of the intestinal tract. Persistant perleche is a good indication of LGS.

I'm very confused. I was in mexico and I had redness and small painless blisters around my lips. A doctor at the hotel said It was "herpes simplé"(oral herpes). Then I came back and told a good dermatologist about what happened. She said that it was more characteristic of perleche that might have been caused by biting into a lot of limes (in the bars, with beer, with all the liquor I had down there), but she never saw the blisters and the crusting. Now i don't know who to believe, the Mexican doctor who saw the outbreak and might not have known what perleche was, or the reliable American dermatologist who heard the story but didn't see the actual infection. Do I have perleche or Oral herpes?

Hi, Alex.
I have never heard of blisters from perleche. I have never heard of painless herpes blisters.
Both of these conditiona are of the "see one, do one, teach one" variety. In other words, once seen they are VERY easy to diagnose by any competently trained physician.
I guess you will just have to wait to see if it will recur and THEN see your local dermatologist WHILE it is active.

I to have fissures around the corners of my mouth.I've had the problem since '91.One side will flare up and crack for about a month, then the other side will flare up.This is a painful, uncomfortable condition.I haven't been able to find any cure .I use a prescription steroid cream called HALOG to try and control these flare up's.I wish I had some more info .

Hi, Mark.
If you have used the Halog ever since '91, you now may have the problem BECAUSE of the treatment. Tell me that this isn't so!
If the above isn't so, you can probably cure this by starting two capsules of "Balanced B Vitamins 100" twice a day. if this is going to help, you will see great progress in about a month. After your lips are as good as they get for a month or so, you can cut the dose in half & see if they stay great. If they do, wait a month or so & cut the dose in half again. I doubt that you will ever be able to get along with less than one of these a day for the rest of your life. You can try it &, if the problem has not recurred in a year or so, be happy. If it does recurr, at least you know how to resolve it.
Let us know how you do.
If you HAVE been using the Halog for all these years, let us know & perhaps I can offer you some other options to consider. Either way, the Balanced B will help. It is just that it may be too late to cure it if you have used the cortisone that long.

Hi, Mark.
Youu would have to take a h--l of a lot of balanced B to have any side effects.
At least if you tried it for a few months, you would know if it was related to your perleche.

Dr. Stoll,
Thanks for the advice. The Vitamin B Complex I have taken twice per day for 8 weeks has certainly helped. The white raised bumps at the corners of my mouth are virtually gone.
But I still have uneven skin and redness at the corners. Should this go away as well? Are there particular foods that will help? Should I continue with the same dose of B? Should I also be taking other vitamins? Which ones?
Thanks for your help,
Pete

Hi, Pete.
I would keep up the same dose until no more progress for a month OR till the problem has been cleared up for a month.

Curious to know Dr. Stoll what could Halog or cortisone in
general do to impair healing of this condition? And if this
was the case, what options are there out there. I've had
this condition going on two years and now following advice
on B vits. and also iron supplements. When I first got this
evil demon, I used OTC cortisone for a year to control
dryness. Once again, waiting for a cure....Thanks Dr.

Hi, William.
Remember, persistant perleche is ALWAYS related to LGS (It is the beginning of the intestinal tract.)
If the B vitamins were going to resolve this they would have done so within 2 months at the most. I have never heard of the iron/perleche connection.
The chronic use of cortisone will eventually destroy your dermis and you SURELY don't want that. That would cause permanent deformity of your face that MIGHT be resolved by plastic surgery.
ALL cortisone products' lables must clearly state that this is to be used for short times only (several months at the most).
To explain why this stuff could delay healing you would have to read any Pharmacology book on the subject. Just ask your library to find you a modern book and look it up. It should be no more than a few pages.
Let us know what you learn.

Hello again. Thanks for the warning about cortisone. One
more question for now, what are those little white/clear
bumps that form these raised fissures called? I have
noticed that if I put anything on the corners of my mouth,
cort., nizoral, any moisturizer, they get bigger or the
fissures get bigger and more pronounced and after i shave i
have to hide for a day so no one can see my mouth. thank the
lord for facial hair, it definitely helps in hiding this.
Thanks again Dr. Stoll, you have been the only doctor that i
have found that has been interested in this condition. The
last three doctors i saw said "Duhhhhhh" So
frustrating

Hi, William.
Of course, the docs who said "duhhhhh" didn't charge you.
These white spots are the herniated linings of the ducts to the glands involved.
Let us know if you have questions as you get at resolving your LGS.
