armit cyst removal

I posted a message a month ago re: my armpit lump. To remind you- I had gone to a surgeon and we had it out because he told me there are no ducts in the armpit area and that I didn't have a cyst. Then I went to another surgeon and he removed a cyst the size of my thumb and said a "nubbin" would develop after a month and I would have to get it removed.
Well, i admit i didn't ask enough questions and honestly it was because I was afraid he would react the same way the first surgeon did. So, I went back last week and he said it's ready to be removed and I am to go to a surgical center on Monday. He said depending on the amoutn of bleeding I will either have a drain or get stitched up.
I was shocked because I thought the removal was going to be like the first one- done in his office with a 1 inch incision, no stitches, etc.. I'm a little confused now and worried. I know I should be asking him these questions but like I said, I'm a little afraid- he's a nice man but the other surgeon really affected me. I read int he archives that a cyst needs to removed in its entirety. Why did he remove the cyst but still has to remove the "nubbin?" does this mean he did not get the whole cyst? Will the initial removal affect the removal on Monday? He said the cyst had ruptured so perhaps he removed the "bad" part? Would the nubbin have formed if he removed the whole cyst or does the nubbin always form afterwards? As you can see, I'm very confused! And why does this removal require a surgical center when the first removal was done in his office? The nurse said the first removal was actually called a draining- I thought a draining was just to get pus out- I did not know it included removal of a cyst. I guess basically, I would like to know why removing the "nubbin" is different from removing the cyst. Your help and time is greatly appreciated. Thank you.

Hi, Ruth.

Look at any microscopic sagittal section of skin and you will see how many thousands of "ducts" there are in just the area under your arm. You must have misunderstood his statement that there were no ducts in that area!

I have never had a "nubbin" develop after removal of a sebaceous cyst--unless I missed part of the cyst wall. It is nearly impossible to get it all if the cyst has ruptured--which he admitted had happened.

You will be fortunate to not develop new cysts along the incision from the damage to the ducts of other sebaceous glands in the area. I see no option for you now that the first cyst had ruptured--unless you are willing to live with the nubbin.

Of course the nubbin is just another cyst and, if the doctor would be careful to remove it without rupturing it, it would be also cured. In that case, if this were me, I would let it mature for a few years and then get it removed without rupturing it. That would take a very short incision and the cyst could be delivered like a baby. That would risk damaging many fewer healthy ducts.
