Pilonidal Cyst Trauma

Hi Dr.Stoll,
After researching the internet on Pilonidal Cysts, I came across your BB and found many similar experiences. In May 2001 I had my first surgery completed in Boston and the doctor told me that she would take the whole (3) cysts out and I would experience some pain with the healing but should be ok, well since then its been hell on earth. My first surgery did not heal and I went through the summer with it open and very painful. I went to another doctor in Sept 2001 and he stated that there was still another Pilonidal sinus present and that the 1st doctor did not take all of them out. In November 2001 I had my second operation and I started to heal great until Jan.2002. The length of the wound was 3 1/2 inches with a depth of 2 inches. In Jan. 2002 the healing process stopped and the wound decided to stay open about 1/2 of an inch at the bottom. I went back to the doctor and he stated that plastic surgery might help and consider it but the risk of not healing goes up everytime you have sugery. I decided against it. In March 2002 I had a laser surgery to burn the hair off around the wound and this started to promote healing a little until I went back in May 2002 and the technican burnt my scar and opened it a little. The pain is not that bad but I cannot touch the wound at all, it is very sensitive. It still drains green and bleeds every week. I am seeing different doctor on Oct 7 2002. Can you recommend what I should do? I appriecate your advice.

You are describing what 50% of people, who opt for surgery for this problem, experience. In MY opinion, the risk is too great to take and, except for the most desperate cases, I personally would opt for a conservative management of the condition for life. Now that you have had the surgeries, your options are seriously curtailed.

Spend a few hours on the archives about this and then let us know what you think of your options at this stage. My bet is that, had you known all this prior to your first surgery (something the surgeon is legally required [Informed Consent] to tell you prior to any surgery) you might not have done it.

I have a few suggestions for your current situation that might help. Since what is done is done there is no need to go batting yourself on the head as to whether you made the right decision on surgery or not. Trust me I've been there!

For me I would pick surgery even if it takes several and I've had two. I just could not live with my extremely painful flare-ups.

The only way to remove the cyst from your body is with surgery. The problem with surgery is that if you have alot of cysts inside it could be very hard to remove all the cyst and thus end the problem. That is way for some people surgery doesn't work. But for many surgery is succussful.

There is no documented research that supports a 50% recurrance rate for people who have had surgery. It doesn't mean it's not true but it also means it may not be true. I've found no research that supports any long term recurrance rate so if you hear one just keep an eye open. The only recurrance rates out there are for post op infections.

Now on to what you can do now. First you can try a wound healing center. Alot of people use these centers when they have problems with the open wound healing. These centers specialize in healing wounds and may have ideas that a regular doctor does not have.

I'm not sure what kind of surgeon you've used in the past but researching for a good surgeon is very important on the Pilonidal as you've learned on this site and many others every doctor and person has a different idea on how these cyst form, grow, and should be treated.

That just shows you how messed up the medical field is on the Pilonidal you've got to make sure the doctor has done is homework. And from my experiance the best doctors for this are Colon/Rectal Surgeons or General Surgeons. Again you've got to test them.

There are a few surgery options. A good site to read that give you all the surgery options is www.pilonidal.org. This site is just about Pilonidals and there is a discussion board that has alot of fellow suffers that can help with ideas and suggestions. Not to mention a great site for information on the Pilonidal.

I'm just one person so I suffer, and it's my choice to use whatever name I want.

Does it make you feel important and smart to cut down another who is trying to help someone else?

Careful how you judge another because one day you may sit before the pearly gates and I'm sure you'll be turned away because you made a grammer mistake way back when.....let's see that was... what comes around goes around.....remember that goes for you too.
