I am in the middle of treatment for a so-called pilonodial cyst. After my surgery to remove the cyst, I came out with a BIG HOLE IN MY BACKSIDE. I walked around for a month with this hole sucking and blowing air. The whole time, my doctor was telling me that the incision would just have to heal. After a month, I got a second opinion. I ended up with a large fistula that had to be surgically repaired or should I say opened. My surgery was on 12-31-02. It is now 05-12-03 and I have healed all up but for one hole around my tailbone, about the size of an eraser. I told the colon and rectal surgeon that when I passed gas, air came out of the hole around my tailbone. I was told it was fine. I'm thinking I wasn't born with 2 holes back there, how can this be fine? So, I went and got another opinion, and this other doc opened the rest of the fistula that the other doctor missed, in his office. It seems to be healing well, and I still have air coming out of the hole. But, I have such excruating tailbone pain and with the air coming out of the hole, could I still have another fistula. My CT was negative but the MRI suggested a suspicious fistula. This new doctor seems to think that he needs to replace the fatty tissue around my tailbone and do something with some kind of flap, to protect my tailbone. He said something about a pick line???? I think that is something for antiobiotics??? Please give me your advise, if you have some. Thanks so much!

It surely is hard to tell from here but I do have the advantage of "Monday Morning Quarterbacking". Having said that, in my opinion, I think your new doc is right and he (and you) will be fortunate if he can repair the damage left by the first docs.
These can be really hard--to the point of being impossible--to deal with so the fact that the first 2 docs failed is not unusual. Really, my only complaint about them is that they tried to deny the failure when it happened.

You are right!!! Yes, they BOTH FAILED miserably. This new doc seems to be on the ball with things. He has recommended doing the flap or something like that. I don't fully understand it. But, I go tomorrow and see him and I guess he will make his recommendations. He seems to be doing the right things though. I'm just a little scared thinking about another surgery. Especially one that will possible require a 3-5 day hospital stay. He said something about a pick line, whatever that means. I assume if it causes pain, it can't be good.
I don't know if you have a site for pictures. But, I made my husband take pictures from my 1st surgery and 2nd. They are pretty interesting. My fistula was about 6 inches deep and about 6 inches long.
I'll keep the site posted on the flap thing. :)

Just posting a follow-up after my appointment last week...
As this wound is healing, another hole is visible inside the initial incision... I definetly have another fistula. Walt, do you know ANYTHING about fistulas? I don't have Chrone's Disease.... In addition, this fistula has gone all the way through my sphincter muscle. I know that when I have to use the restroom, I barely make it to the bathroom. I am so scared that I am not going to make it some of the times. I know I need to discuss this with my colon and rectal surgeon, but are there other alternatives other than another surgery? Any information would be of great help! Thanks.

Oh, by the way, what is a pick line? Anyone know? I know it is some sort of IV. But, why is it called a pick line and JUST WHERE does it go?

Fistulas through the anal sphincter are BAD news and surgery is the only option, to my knowledge.
What was your first surgery for?
