Perioral dermatitis/rosacea/acne

Hi Dr Stoll!

I am a 26 year old female who was lucky to have very clear skin until I was about 24. At that time I was going through a HUGE amount of stress when starting graduate school and had acne problems. Those went away, and in the two years since I was lucky to have clear skin again. I have been told before that I have rosacea, but NEVER had trouble with pimples or flushing etc, just rosy cheeks.

About four months ago, in june, I visited a dermatologist for what he diagnosed as perioral dermatitis around my nose and on my chin, and also for psoriasis on my elbows. I used Cleocin-T on the perioral and it did WONDERS and an emollient for my elbows, that is gone too. I started to notice a few more pimples than normal and a seemingly rosacea related breakout on my cheek in july after stopping the emollient.

At my follow-up appt in July, the doctor prescribed Tetracycline for the acne and rosacea bumps. I switched to minocycline after two and a half weeks because it didnt seem to be working...in fact, I had MORE ACNE and little bumps on my face on the tetra than before I went on it! He switched me to minocycline and I have been on that for five and a half weeks, and my condition has gotten a LOT worse. I am also using noritate on my cheeks at nite.

I went back to the derm yesterday and he told me to get off of the minocycline, and also prescribed cleocin-T again, twice a day on my whole face, and also to maintain the noritate. He told me that I have perioral dermatitis again, but this time on the sides of my face, my chin, around my mouth and on my forehead.

I am curious.....can the tetracyclines (tetra and minocycline) CAUSE perioral dermatitis? It seems to me that since I have been on the meds, 8 plus weeks, that my condition has gotten SO MUCH WORSE! I suspected it was the antibiotics, but my doc says it would be rare if that was teh case because that is how they treat perioral.

Any thoughts on this? Does anyone know of antibiotics CAUSING perioral dermatitis?

I should also say that this summer was EXTREMELY stressful for me, and I am currently SO STRESSED about my condition. I am MISERABLE. In addition, in june I had an abdominal injury (ruled a muscular injury) that has come back again....coupled with further progression in my condition on my face. I am going to have a CAT SCAN next week.

Any thoughts on this would be GREATLY appreciated! I am BESIDE myself with this mess! I just want to have clear skin again!

Since all three of these conditions are so intimately related, the solution to them is the same.

Since LGS is intimately related to all 3, and LGS is most commonly caused by stress-effect storage.............

Antibiotics can produce temporary benefits but make LGS a lot worse in the long run. SO, it is not surprising that your condition might get worse with antibiotic therapy.

MY suggestion would be the essential oils on the home page while you are dealing with the LGS.

Thank you so much! I really appreciate your time! I am going to read up on the LGS in the archives and also try to start SR. I am CONVINCED that a LOT of what is happening is STRESS RELATED.

One more question.....

Would you suggest stopping the antibiotics? Would it be more beneficial to try all this stuff without them?

How much have the antibiotics helped? Even though they might give temporary relief, they ALWAYS make the problem worse in the long run.

They havent helped AT ALL. In fact, when I went on them I barely had any acne (went on them for rosacea, apparently the proper treatment for rosacea) and now I have a TON of acne.....not typical acne either...little pinpoint red marks that never come to a head......and they are on parts of my face that I have NEVER had a blemish on before in my life......

In fact, since I have been on the AB, my condition has only gotten worse...the redness has gone away, but I wasnt really red to begin with!

I am concerned I will flare getting off the AB, but I think, in the long run, I will be better.

What does "long run" mean.....they will make the condition worse "in the long run". am I already at that point?

Sounds to me like you have nothing to lose by chucking them.

Your SR will take 3-6 months to start helping and about 12 months to get the maximum benefit (See the other things that will help in the LGS archives.)

If the antibiotics were going to help you should see benefits within a week.

you mean when my doctor told me I needed to try the AB out for six+ weeks before I would see a benefit he was probably wrong? I should have seen a benefit in one week?

WOW. I have been on meds for almost 9 weeks now, and seen no improvement. It only got worse in the first week! I should have taken that as a sign and gotten off of them.

I will keep you posted on my progress. thanks for the info and your time!

You are learning why it is SO important for anyone to take the time to understand any chronic condition BEFORE undertaking any physician's "practicing".
