tendonitis/arthritis

I need some help. In July of 2000, I sustained a severe ankle sprain while at work. After one month of the ankle not improving, I went to an orthopedic surgeon who said that I would need surgery in order to continue my career as a firefighter. So, in October I under went a ligament repair and three months of therapy. After all of that, my ankle had several "rolling" episodes. Of course, I went back to my surgeon, who examined it and proceeded to tell me that if I did not undergo a reconstructive procedure, I would have to change my career. Naturally, I had the surgery followed by three months of physical therapy, again. Since then, I have begun to have constant pain, that is worsened by walking and basically anything. The pain is so bad at times, that I am nearly brought to tears. I returned to the doctor, who prescribed physical therapy and Vioxx. The Vioxx has not touched the pain, and therapy has not reduced the inflammation nor has it reduced the pain. My physical therapist believes that it may be tendonitis (according to her, that would be the best option for me). Whereas, the doctor is leaning more on arthritis, even though the x-rays show normal (he has also ordered a bone scan, but it has not yet been performed). I am only 23, and need help!!! What would the outlook be if it is tendonitis? Arthritis? What would the long term prognosis be?
