TMJ

Dr. Walt:

From auto accident 7/1995, severe TMJ symptoms. Arthrogram 4/96 shows rt.side large fixed anterior subluxation w/ partially limited opening and left side moderate sized partially reducible anterior subluxation, with perforation, with slightly limited motion. Received conservative treatment until 10/1998, at which time a maxillary osteotomy was performed along with orthodontics. Went through several different bite plates/mouth splints to no avail - remove them in my sleep. Still daily chronic pain, inflammation, cluster headaches, sleeplessness. Follow soft diet, wide opening restrictions, relaxation techniques, muscle relaxants at p.m. Dental visits a nightmare, therefore, skipped for last two years. Now critical work needed to be completed. Cannot find a prosthodontic dentist who will complete work without my having a partial or total joint replacement surgery first. Desperate to avoid another surgery that promises 0% guarantees of symptom relief. Do I have any other options?

Hi, Angela.

Did your "conservative management" include the most important thing for this: SR?

As you must know, this is a very advanced case and your options are all gone. STILL, SR will help you increase the success of any surgery.

Surgury usually brings worse(mega) probs. There are tons of horror stories proving this. Do some searches on the net. With the SR(a good idea) you might want to try a NTI for the bruxing, you can read about it at (I THINK) nit-tss.com, if not just search it. More and more docs. are ditching the splints for this device. I have one and it works great, splints I have no use for. The SR really helps it to work, you have to break the bracing habit.

Of course you can see why the actual cure would not be popular among most dentists: it costs nothing whereas the intervention stuff brings in mucho dinero.
