Dr. Stoll,
You have answered several questions for me on TMJ. I am still trying to decide whether I should have surgery. I am leaning towards having it because of the pain.
Could you tell me what this MRI report means? Thanks -Lisa
The left TMJ shows no evidence of any anterior translation of the left condylar head. There is irregularity in the contour of the left condylar head and there is no detectable meniscus in the left TMJ. Postoperative fibrosis is present since there is no anterior translation of the left condylar head.
The right TMJ has an anteriorly dislocated meniscus. This reduces on open mouth positioning. There is good antereior translation of the right condylar head. The very small compartment of the right TMJ is not out of keeping wiht previous surgery. There is also flattening deformity of the right condylare head. This is postoperative.
Impressions:
1. An abnormaal MRI scan suggestive of at least fibrous adhesions of the left TMJ with a nonexistent left articular meniscus. There is no anterior translation of the left condylar head.
2. A small but reducing anteriorly dislocated articular meniscus of he the right TMJ. No effusion ofr suspicious perforation of the right TMJ.

Lisa,
This IS very late in the game.
No time for recriminations for your previous professionals.
Surgery and prosthetic menisci may be your only option since the lateness of the diagnosis has severely limited them.
HOWEVER, even now, eliminating the bracing that caused this will make any postoperative successes more successful and less painful. See the bracing archives as to how to do this.
Hope this helps.
Walt

Thank you for taking the time to look at this for me. I am working on the bracing. Trying to do some relaxation as SHAPE has suggested. I guess I will just need to pray hard before I do anything.
