Dr. Stoll, I have had an ongoing battle with fungal toenails. Ironically, I got a bacterial infection in one nail over the winter, resulting in the nail falling off which got rid of the fungus as well. Now I have another bacterial infection in another toenail...you know what I am thinking, I am secretly hoping this nail will fall off. One less nail with fungus. I have not re-developed fungus in the other nail that fell off and re-grew. I had a second, minor bacterial infection in my baby toenail, but after it drained it just healed and the nail never fell-off. It still has fungus. Part of the reason I get the bacterial infection, I believe, is that the darn fungus-infected nails are so mis-shapen that when I file it down, cut it, etc. I tend to nick the nail bed. What can I do to encourage this latest nail to separate from the bed and fall off!!! I have tried every single remedy posted here and nothing works. It has been six months and no change. The only thing that has gotten rid of these damn fungus-infected toenails is for them to become infected, drain, the nail to separate and finally fall off and have a new one grow in. Its pretty bad when you are actually hoping for bacteria to take over. Again, what would encourage this nail to separate and fall off, I am tempted not to take my antibiotics just to encourage a worse infection!!!! Helpp!!!

Hi, Josephine.
One of the long standing conventional treatments for chronic onychomycosis has been to tear the toenail out by the roots and let the new nail grow back in normal. It helps to use the vinegar till the new nail is seen peeking out.
The only problem with this is that, unless the surgeon is knowledgable, skilled and careful the nail bud can get damaged and then no nail will ever grow again or it may be grow deformed for the rest of that person's life.

I'm curious, (and at the end of my rope) is this procedure considered cosmetic or necessary treatment which could be covered by insurance. I was happy the last nail fell off on its own, but I do not seem to be that lucky this time. It seems to be just healing (the bacteria) but the nail and fungus remain.
